There are a number of both administrative/ business forms as well as forms for production and publishing which are regularly used by the student journals.  As these forms are occasionally updated, it is important that you be using the most recent versions.

Administrative and Business Forms

• Entertainment Certification Form:   order or get reimbursed for any food or drink related to specific journal meetings or events
• Payment | Reimbursement Form:   order or get reimbursed for any supplies, goods, or services
• Travel Reimbursement:   request reimbursement for any travel related expenditures

Publishing and Production Forms

• Author Copyright Agreements:   required for each publishing author for copyright, responsibilities and expectations, remuneration
• Offprint Order Forms:   captures author offprint order information:  quantities and shipping addresses
• JCI Order of Materials Form:  must be submitted to the printer along with article files
